Achievement still locked. (Destiny)
So it turns out I never did get flawless raider. I could have sworn I had, but that's not what my account is saying. If I recall correctly, it's fairly easy to get in the Crota raid. Would anyone here be up for this? I haven't touched Destiny in Months, so I'm not sure how solid I'll be.
Force it to unlock
If you're sure you completed it, maybe the achievement didn't unlock. Here's a trick to forcing it to unlock:
In Destiny, log into the character that completed the Flawless run. Then go to the Menu > Settings > Sign Out > Switch Profile
(Notice it's not "Change Character")
Then sign back in with your profile and it should unlock. I had this same issue with the Flawless Raider achievement and this fixed it.
This also fixed mine and 15 achievables in Madden 16.
