People in the Bungie West office receiving and sending models of Konoko back and forth with Bungie (main?) and her clothes were getting more and more torn and disappearing slowly. Eventually West decided to keep her fully clothed if I remember correctly.

…and then in the game she jumps into a vat of green liquid completely naked.
