Do we have to earn one for each weapon and piece of armor we have to buy?

Will I have to go up a rank each time I want to buy something?

And do I get any commendations for all the ranks I've already earned?

My guess: you get exactly as much credit for that as you get for the experience already put into your exotics. I'm not bitter.
