IIRC there were two components for generating the identifier; tenure (Mythic, Legendary, Heroic) and forum disciplinary record. "Exalted" meant no warnings or bans on file.

-- Steve had to bow out soon after those started, though, so he's not terribly clear on the details.

edited to add; just saw a link to the Bungiepedia article that seems to cover the issue in more detail. Looks about right, but again it's been a looong time so memory's a bit hazy on the subject.
