Crota normal mode glitchtastic after patch? (Destiny)

We also had a glitch where the sword would disappear when we tried to pick it up... That was an insta-wipe issue.

Yeah, this has happened a bunch of times on my raids the last two weeks. It still happened before, but not nearly this often.
