It's not really a bug, it's just not explained well. If you press the button to restart the mission (X for us Xboxers) on the screen where it actually tells you "Press {whatever} to restart mission," you're only resetting progress on the easiest difficulty. You have to press that button when you select your difficulty level to reset progress for that difficulty.
