What in the actual hell. (Destiny)

I have an adapter. It works. So did just plugging into the controller... yesterday... All the hardware is fine. I can dig down and find the volume settings and turn up the "mic monitoring" to the point where I can absolutely hear myself / hear taps on the mic. It's either Destiny has gone completely bonkers or (more likely) the Xbone's software is a piece of crap and I've somehow caused it to stop hearing the mic in some rare cases or some crap like that...
